CAR or CA$H for Healthcare Lottery With 50/50 Raffle  

  

REQUIRED INFORMATION  

The CPDMH Foundation (Foundation) was approved in 2019 as eligible to apply  for a provincial licence. 

Established in 1994, the Carleton Place & District Memorial Hospital (CPDMH)  Foundation is a registered charity dedicated to raising funds for the Carleton Place  & District Memorial Hospital in support of providing the best healthcare possible  to the residents of Carleton Place and surrounding communities. Each year the  CPDMH Foundation commits to purchasing modern and up-to-date patient care  equipment which is needed for the hospital. In addition to providing vital  equipment, the CPDMH Foundation also raises funds to support staff education  and training, high school bursaries, various specialized equipment and program  needs and upgrades to the hospital facility. 

As per our by-laws, By-Law #1 6.01 and 6.05, and, Letters Patent (Section VII), we  are to conduct individual fundraising initiatives and we may conduct activities in  partnership with other charities and non-profit organizations.  

This WIN2023 Car or Ca$h for Healthcare Lottery and 50/50 Raffle is run in  partnership with the Almonte General Hospital (AGH) Foundation. All net  proceeds are split 50/50 with our partnering hospital foundation.   

ELIGIBILITY 

  1. Ticket purchasers must be located in the province of Ontario at time of purchase. 
  2. Purchasers must be at least 18 years of age. 
  3. Tickets cannot be purchased for or in the name of persons less than 18 years  of age. 
  4. The following persons are prohibited from purchasing tickets in the WIN2023 Car or Ca$h for Healthcare Lottery and 50/50 Raffle:
a) CPDMH Foundation staff and Board of Directors;
b) AGH Foundation staff and Board of Directors; 
c) Bean Chevrolet Buick GMC Corvette Ltd. Owners and employees;
d) The Carleton Place & District Memorial Hospital Senior Team, the Almonte General Hospital and Fairview Manor Senior Team, and Mississippi River Health Alliance (MRHA) Directors; 
e) Raffle event promoter, Kelly Prescott;
f) Employees of the registered gaming supplier, BUMP a Division of Canadian Bank Note Company Ltd.;
g) And members of the same household of each of the above; and
h) Employees of the Alcohol Gaming Commission of Ontario.

 

TICKETS 

  1. Total value of all prizes for the WIN2023 Car or Ca$h for Healthcare Lottery is $116,021. 
  2. Total value of the 50/50 Raffle is to be 50% of all ticket sales.
  3. A maximum of 23,204 tickets are available for purchase. 
  4. Ticket price for Car or CA$H for Healthcare Lottery are $25 CAD each, 5 for $100 CAD, or $300 for 20, max. of 250 bundles.  
  5. Ticket prices for the additional option of the 50/50 raffle are $10 for 50, $20 for 120, or $40 for 250. *50/50 Tickets can only be purchased with a valid  purchase of a Car or Ca$h for Healthcare Lottery ticket. 
  6. All valid tickets will be included in the Early Bird prize draws, including the early-bird winning tickets, will be included in all additional Early Bird and  the Grand Prize draw.
  7. The player is solely responsible for ensuring they receive the correct ticket denomination and solely responsible for the handling of the purchased ticket(s) after the online purchase or POS device purchase has issued the ticket. 
  8. All players must be over the age of 18 years and be physically located within the province of Ontario at the time of purchase. 

PRIZES 

EARLY BIRDS:  

  1. The 1st Early-Bird Draw is valued at $2,000 for a grocery shopping spree at  Mitchell’s YIG in Carleton Place, ON. All qualifying tickets for the 1st Early-Bird Draw must be purchased by 12 noon on Friday, April 21, 2023.  
  2. The 2nd Early-Bird Draw is valued at $2,000 for a grocery shopping spree at YIG in Almonte, ON. All qualifying tickets for the 2nd Early-Bird Draw must be purchased by 12 noon on Friday, April 21, 2023.  
  3. The 1st Early-Bird Draw will occur at 1 p.m. Friday, April 21,  2023, at Mitchell's YIG, 455 McNeely Ave, Carleton Place, Ontario K7C 0A1.
  4. The 2nd Early-Bird Draw will occur at 1 p.m. Friday, April 21,  2023, at Jonsson's YIG, 401 Ottawa St., Almonte, Ontario K0A 1A0.
  5. The 3rd Early-Bird Draw is a Youth 4-Wheeler and accessories donated by the Throttle Powersports Almonte valued at $5,174.96. All qualifying tickets for the 3rd Early-Bird Draw must be purchased by 12 noon on Friday, May 5, 2023. 
  6. The 3rd Early-Bird Draw will take place at 1 p.m. Friday, May 5, 2023, at  Throttle Powersports Almonte, 4692 March Rd, Almonte, ON K0A 1A0.
  7. The 4th Early-Bird Draw is a Louisiana Grill donated by Beckwith Butcher  valued at $2,825. All qualifying tickets for the 4th Early-Bird Draw must be  purchased by 12 noon on Friday, May 19, 2023. 
  8. The 4th Early-Bird Draw will take place at 1 p.m. Friday, May 19, 2023, at The  Carleton Place & District Memorial Hospital, 211 Lake Avenue East, Carleton  Place ON K7C 1J4. 
  9. The 5th Early-Bird Draw is a Milwaukee Tool set donated by Levi Home Hardware valued at $4,003.32. All qualifying tickets for the 5th Early-Bird Draw  must be purchased by 12 noon on Friday, June 16, 2023. 
  1. The 5th Early-Bird Draw will take place at 1 p.m. Friday, June 16, 2023 at Levi Home Hardware, 476 Ottawa St. Almonte, ON K0A 1A0 

GRAND PRIZE:  

  1. The 6th and final draw is the Grand Prize for the winner’s choice of a 1963 Convertible Sting Ray Corvette valued at $100,000 OR $100,000 CASH. All  qualifying tickets for the 6th and final Grand Prize Draw must be purchased by 7 p.m. on Friday, June 30, 2023. 
  2. The Grand Prize Draw will take place at 8 p.m. on Friday, June 30, 2023, at the Pakenham General Store, 2524 County Rd. No 29, Pakenham ON K0A 2X0 

50/50 RAFFLE:  

  1. The 50/50 Raffle prize is 50% of the total gross 50/50 ticket sales. All qualifying tickets for the 50/50 Raffle draw must be purchased by 7 p.m. on Friday, June  30, 2023. 
  2. The draw for the 50/50 Raffle will take place at 8 p.m. on Friday, June 30,  2023, at the Pakenham General Store, 2524 County Rd. No 29, Pakenham ON  K0A 2X0.
  3. All prizes must be claimed within 6 months of the relevant draw date(s) unless otherwise specified by the Carleton Place & District Memorial Hospital Foundation (CPDMHF) and its partners, Almonte General Hospital Foundation (AGH Foundation). 
  4. For groups purchasing (1) ticket, group winners will select one individual to handle the distribution of the prize associated with the winning ticket.
  5. Carleton Place & District Memorial Hospital Foundation (CPDMHF) will not award a prize for tickets which are void. Tickets are void if lost, stolen, unissued, illegible, mutilated, damaged, altered, counterfeited or forged, miscut, misregistered, defective, misprinted, cancelled, produced in error and not recorded in the on-line system, incomplete, not paid for, destroyed or issued, acquired or presented, in, or upon, violation of these Rules, or the Game Conditions. Void tickets are the property of the Carleton Place & District Memorial Hospital Foundation. 
  

TICKET PURCHASE 

  1. All tickets for the WIN2023 Car or Ca$h for Healthcare Lottery AND the  accompanying 50/50 Raffle are sold through BUMP, A Division of Canadian Bank Note, systems and website. 
  2. The official online website for ticket sales is www.Win2023.ca. Tickets sold through the website are provided to the ticket holder via valid email address. Tickets may also be sold via face-to-face sales with Point-of-Sale (POS)  machines at various locations throughout the region, starting midnight on  March 31, 2023, to 7 p.m. June 30th, 2023. Tickets sold through POS devices are provided to the ticket holder via printed ticket only. 
  3. All valid tickets will be included in the Early Bird prize draws, including the early-bird winning tickets, will be included in all additional Early Bird and  the Grand Prize draw.
  4. During the online and POS device checkout process, purchasers have the option to buy an additional 50/50 Raffle ticket. In order to purchase a 50/50 Raffle ticket, a WIN2023 Car or Ca$h for Healthcare Lottery ticket must be purchased EACH TIME. 
  5. A valid ticket is receipt and proof of purchase.
  6. Ticket sales can be purchased by debit or credit card only. No cash purchases will be accepted. 
  7. All ticket sales are final at the time of purchase. No refunds will be issued for any ticket sales. 
  8. In the event that a ticket which is purchased or issued is void, or deemed to be void, the Carleton Place & District Memorial Hospital Foundation (CPDMHF) may, replace the ticket with a new random ticket of the same denomination. At no time will a refund be issued for the purchased ticket. 
  9. Only valid tickets are eligible for the draw. All ticket purchasers must provide the original ticket copy. No photocopies or picture copies of the ticket will be accepted. 
  10. All ticket purchasers must provide an email address, telephone number and a  valid credit card to complete the purchase of their tickets.
  11. Tickets can only be issued to the owner of the credit card and prizes must be  claimed by the owner named on the credit card. 
  12. Tax receipts cannot be issued for ticket purchases.  

DRAW 

  1. Winning ticket number, and name and place of residence of the winner(s) will be recorded and announced immediately after each draw through the event website and social media platforms.
  2. Lottery winners will be notified by a representative of the Carleton Place &  District Memorial Hospital Foundation. Such notification will include information about the prize claim procedure. 
  3. Winning ticket number and name of winner will be broadcast by the WIN  2023 Hospital Raffle System at www.win2023.ca.  
  4. 1st Early-bird draw prize will take place at the respective shopping spree  locations Mitchell’s YIG, 455 McNeely Ave, Carleton Place ON K7C 4S6  
  5. 2nd  Early-bird draw prize will take place at the respective shopping spree  location of YIG, 401 Ottawa St. RR#4, Almonte ON K0A 1A0 between April 3rd and June 29th, 2023.  
  6. The 3rd Early-Bird prize, a youth 4-wheeler, will be announced at Throttle  Powersports Almonte, 4692 March Rd. Almonte ON K0A 1A0 by a representative of the Almonte General Hospital Fairview Manor Foundation.  
  7. The 4th Early-bird draw prize, a Louisiana Legacy Grill, will be announced at the Carleton Place & District Memorial Hospital by a representative of the  Carleton Place & District Memorial Hospital Foundation.  
  8. The 5th Early-Bird prize, a Milwaukee Tool Set, will be announced at Levi Home  Hardware, 476 Ottawa St. Almonte, ON K0A 1A0. 
  9. The Grand prize, is the choice of a 1963 Convertible Sting Ray Corvette OR $100,000 CASH. If choosing the 1963 Convertible Sting Ray Corvette, the  winner must take delivery as awarded at Bean Chevrolet Buick GMC Corvette  Ltd. by a representative of the dealership and Carleton Place & District Memorial Hospital Foundation. The winner is responsible for insurance from the time of delivery/ownership. Delivery of the vehicle is P.O.D., Carleton  Place, Ontario.
  10. The draw for the 50/50 Raffle add-on will take place at the Pakenham  General Store, 2524 County Rd. No 29, Pakenham ON K0A 2X0

SECURITY AND SAFETY 

Policies and Procedures: 

a. Data Integrity and Backup: BUMP, A DIVISION OF CBN, is an AGCO approved provider for the electronic raffle and will be providing the RNG  System. They have advised the Foundation that they have automatic backups for all raffle sales data and that it will be available from their secure servers.

b. System Failures: BUMP, A DIVISION OF CBN, will provide access to the  RNG system. The RNG is web based and can be accessed only by authorized persons through any computer connected to the internet. If the system were to fail at the time of a scheduled draw, all attempts to  rectify the fault would be made so that the draw could be made as soon  as is practicable.

If we are unable to complete the draw due to circumstances such as a power outage, then the draw would be rescheduled to the earliest opportunity and held in the same fashion as outlined above. No future  draws or sales would be held until the delayed draw is completed and the system is repaired.

PRIVACY POLICY: 

Personal information is collected for the sole purpose of completing ticket sales, identifying ticket purchasers and providing information to purchasers about  WIN2023 and future AGH Foundation and CPDMH Foundation joint lotteries. Information, in whole or in part, will not be communicated to parties outside of involvement in the conduct of this lottery.  

TERMS/VOCABULARY 

In these Rules of Play, and in all electronic or printed material related to this lottery: 

a) “AGCO” means the Alcohol and Gaming Commission of Ontario; b) “Win2023 Lottery System” means the AGCO approved electronic lottery system designed by BUMP a Division of Canadian Bank Note and operated by Carleton Place & District Memorial Hospital Foundation for the  conduct of this lottery; 
b)“Dashboard” means the Dashboard component of the computer controlled system designed by Canadian Bank Note for the conduct of this lottery; 

“Draw” means the random selection of a winning ticket number by Carleton Place  & District Memorial Hospital Foundation through the Win2023 lottery system 

“Player” means a person who is eligible to participate in this lottery in accordance  with the Rules and Regulations; 

“Prize” means the consideration or item identified by the Carleton Place & District Memorial Hospital Foundation that is provided to the winner of a draw in return  for the purchase of a valid ticket; 

“Ticket Number” means a unique numerical sequence randomly issued by the Win2023 Lottery System as set out on a Valid Ticket; 

“Valid Ticket” means a Win2023 Lottery ticket issued by the Carleton Place &  District Memorial Foundation that is not void; 

“Winner” means the person who is the bearer of a winning ticket and who  satisfies all conditions established by the Carleton Place & District Memorial  Hospital Foundation to claim the prize;  

“Winning Number” means the number of a valid ticket selected as the winner  during a draw, 

“Winning Ticket” means a valid ticket that bears a ticket number matching the  winning number of a draw.
